At Prestige Homecare Services LLC, we provide Minnesota DHS 245D-compliant Integrated Community Supports (Ics). This service is designed for adults who live in specific community residential settings or multi-unit housing hubs, offering them the right balance of independent living and on-site staff support.

We work alongside clients, families, and county case managers to help individuals manage their households, build meaningful local connections, and stay active in their neighborhoods.

What is Integrated Community Supports (ICS)?

Integrated Community Supports is a waiver service (CADI, DD, BI) that provides support and training in up to four core areas of a person’s life. Our staff assists clients directly within their living environments and out in the local community to build real-world skills.

1. Community Participation & Inclusion

Living independently means being part of the world around you. We help clients break down social isolation by finding local events, joining community groups, and building lasting relationships.

  • What we do: Assist with scheduling social activities, identifying community resources, and teaching community safety awareness.

2. Health, Safety, and Wellness

We help clients maintain a healthy lifestyle and ensure their living space remains safe and secure.

  • What we do: Provide reminders for medications, assist with scheduling medical appointments, set up exercise routines, and run regular home safety checks.

3. Household Management & Daily Living

Our staff works with clients to keep their homes clean, organized, and running smoothly.

  • What we do: Teach and assist with grocery shopping, cooking nutritious meals, managing laundry, and handling regular household chores.

4. Service Coordination & Communication

We help individuals find their voice, understand their rights, and manage their daily responsibilities.

  • What we do: Assist with reading mail, filling out community paperwork, setting up transportation, and communicating effectively with care teams and landlords.
